
Psychological Evaluations
Matthew is able to conduct psychological evaluations in most cases. This will consist of us meeting for 90 minutes to go over your concerns and specific circumstances. Matthew will use a standardized form to make sure that all necessary components are addressed, and he may ask clarifying questions or request additional information depending on your responses. Once we have completed the clinical interview, Matthew will go over his findings with you and give you the chance to ask any questions that you may have. If a form, letter, or formal diagnostic report confirming a diagnosis is needed, please allow up to a one-week turnaround time. Note that Matthew does not currently conduct forensic, custodial, or Department of Transportation (DOT) evaluations. Note that Matthew is currently under supervision for his LCSW, so all reports will need to be co-signed by Matthew's supervising LCSW. Disability Accommodations for College
Please be sure to have any forms or letter requirements from the college/university ready for your appointment if possible. If you have used accommodations in the past through a 504 or IEP, be ready to discuss these accommodations with Matthew as he will need to know what has and hasn’t worked for you in the past.
Outside Diagnostic Evaluations for K-12 504s/IEPs
Matthew is able to serve as an outside evaluator for individuals seeking a diagnosis or second opinion so that a child can access accommodations within the K – 12 school system. Matthew has been teaching at the K – 12 level as an online adjunct high school teacher since 2020 and holds an M.Ed. in special education, thus he possess a thorough knowledge of what needs to be established to obtain a 504 or IEP. This evaluation will include a report with Matthew's findings in addition to recommended accommodations.
Bariatric and Gender Confirmation Surgery
Matthew is pleased to offer psychological clearance evaluations for individuals seeking bariatric or gender confirmation surgery. Note that each surgeon and hospital is different in what their requirements are. Some require a form while others require a letter that addresses specific components. Please be sure to have this information available prior to your appointment if possible.

If we did not address your specific situation and you’re still interested in, or need, a diagnostic evaluation, feel free to email Matthew and he will determine if he is able to assist you. If not, he will refer you to a provider that may be a better fit for your needs.
